Where is this from...?

It's been a long time.

When I was sticking around wiki, I saw this.

In this scan, character's name "Ayaka" comes out but, I can't figure out more than this. :?

The picture had already attracted a question and answer. Go to the Images section and look at the result of the comment Anyone knows where these are from~?
